The Individual Identification Number (IIN) and other data I have provided for the conclusion of the Public Agreement with KaR-Tel LLP (BIN 980540000397) are accurate. By submitting my personal data for number registration, I hereby consent to KaR-Tel LLP’s Policy regarding the collection, processing and protection of subscribers’ personal data and authorize the collection and processing, including the transfer of personal data to third parties and the cross-border transfer of my personal data, in accordance with the requirements of applicable law, the aforementioned Policy, and the List of subscribers’ personal data collected by KaR-Tel LLP.

The dissemination of personal data in publicly available sources of information is not envisaged.

By submitting my personal data for number registration, I accede to the Public Agreement, consent to receive SMS and other marketing and promotional communications from KaR-Tel LLP and its partners, and agree to the activation of the roaming service and to receiving notifications regarding roaming terms. I have read and agree with the text and terms of the Public Agreement, the applicable tariffs for services, as well as the command `*120*601#` to disable the roaming service.

I consent to KaR-Tel LLP photographing me and my documents for processing for the purposes of identification, provision of telecommunications services and other related services, and the performance of service operations after verification of me by photograph, including through the use of information technologies.

I confirm my consent that, when identification is carried out using biometric data, my personal data may be obtained from the state database “Individuals” for the following purposes:

• identification of the subscriber;
• automation of the process of populating and correctly entering the subscriber’s personal data into KaR-Tel LLP’s systems.

I have been informed that if I do not consent to the processing of my personal data, services cannot be provided to me in full.

This consent takes effect on the date my personal data are submitted for number registration and remains in force for the period necessary for the purposes of collection and processing of personal data, unless otherwise provided by the laws of the Republic of Kazakhstan.
